A sudden jerk of an arm, a kick of a leg, a twitch of the fingers, or a brief whole-body movement can be surprising when your baby is asleep. In most cases, these movements are part of normal infant sleep and neurological development.
But babies can make several different types of involuntary movements, and they do not all mean the same thing. Understanding the difference between ordinary sleep twitches, the Moro reflex, benign neonatal sleep myoclonus, and movements that may require medical evaluation can help parents understand what they are seeing.
Why Do Babies Move So Much in Their Sleep?
Infant sleep is neurologically very active.
Newborns spend a large proportion of their sleep in active sleep, the infant form of REM sleep. During this stage, babies may move their arms and legs, make facial expressions, move their eyes beneath their eyelids, and show changes in breathing. Brief twitches and jerks are also common.
These movements may also have a role in neurological development. When a baby’s arm or leg moves spontaneously, sensory information from the muscles and joints travels back to the developing brain. Researchers believe that this repeated interaction between movement and sensory feedback may help shape the neural circuits involved in sensorimotor development—the ability to coordinate movement with information about the body’s position and movement.
So, some of the tiny movements we see during infant sleep may be part of the developing brain learning how to control the body.
What Is a Sleep Twitch?
A sleep twitch is a brief, involuntary movement that occurs while a baby is asleep. It may involve a finger, hand, arm, leg, foot, or several parts of the body.
These movements are often very brief and irregular. They may happen shortly after a baby falls asleep and can recur several times. If they occur only during sleep and the baby otherwise appears well, they are usually considered normal sleep movements.
The medical term myoclonus describes this sudden, brief, involuntary muscle contraction. Myoclonus is a description of a movement rather than a diagnosis: it can occur as a normal physiological phenomenon, but it can also be associated with neurological conditions.
The circumstances surrounding the movement therefore matter.
Is It a Twitch—or the Moro Reflex?
Not every sudden movement in a baby is a sleep twitch. Sometimes what parents describe as a “jerk” or “startle” is actually the Moro reflex, one of the primitive reflexes present in newborns.
The Moro reflex is an automatic response to a sudden stimulus, such as a loud noise, sudden movement, or the sensation of losing support. A typical response involves the baby suddenly extending and opening the arms, spreading the fingers, and then bringing the arms back toward the body. The baby may cry afterward.
The reflex is present at birth and gradually becomes less prominent as the nervous system matures. It is therefore different from sleep myoclonus: a myoclonic jerk is a brief muscle contraction that can occur spontaneously, while the Moro reflex is a more organised response to a stimulus.
What Is Benign Neonatal Sleep Myoclonus?
Some newborns experience more pronounced or repetitive jerking during sleep. This is known as benign neonatal sleep myoclonus (BNSM).
The movements can affect one or both sides of the body and may occur in clusters, sometimes looking surprisingly similar to a seizure. A key characteristic is that the movements stop when the baby wakes.
Despite the appearance of the movements, benign neonatal sleep myoclonus is not epilepsy. It is generally a self-limiting condition and resolves as the infant’s nervous system matures.
Could a Fever Cause Twitching?
Fever can be associated with involuntary movements, but it is important to distinguish between shivering and a febrile seizure.
A child with a fever may shiver or have brief trembling movements as the body responds to a change in temperature. A febrile seizure, however, is a seizure triggered by fever. During one, a child may become stiff, shake or jerk, become unresponsive, or show changes in breathing or skin colour.
Febrile seizures generally occur in children between 6 months and 5–6 years of age, most commonly during the early part of a febrile illness. They are not the same as epilepsy, and most are brief and do not cause long-term harm.
A very young baby with a fever is a different situation. A temperature of 38°C (100.4°F) or higher in a baby younger than 3 months requires prompt medical assessment, regardless of whether febrile seizures are present. If your newborn has a fever, call your healthcare provider immediately.
Could Repeated Jerks Be Infantile Spasms?
Some seizure disorders can initially look like ordinary jerks. Infantile spasms, also called epileptic spasms, are one example. They usually begin during the first year of life and may appear as sudden, brief contractions of the head, trunk, or limbs.
A particularly important feature is clustering: several spasms may occur one after another, sometimes only seconds apart. They can occur around sleep transitions, particularly shortly after waking.
Infantile spasms require prompt medical evaluation because early recognition and treatment are important for neurological development.
What About Benign Familial Neonatal Seizures?
Benign familial neonatal seizures (BFNS) are a rare genetic epilepsy syndrome in which otherwise healthy newborns develop seizures, usually within the first days of life.
Despite the word “benign,” these are genuine epileptic seizures and should not be confused with benign neonatal sleep myoclonus. A family history of seizures beginning during the newborn period can be an important clue.
Suspected neonatal seizures should always be assessed by a healthcare professional.
What About Periodic Limb Movements?
Some babies may make repeated movements of their legs or arms during sleep. These can sometimes resemble periodic limb movements, which are repetitive movements occurring at relatively regular intervals during sleep.
However, seeing repeated movements does not mean that a baby has Periodic Limb Movement Disorder (PLMD). PLMD is a clinical diagnosis involving more than simply observing a baby’s legs move during sleep and is much less common than ordinary infant sleep movements.
For this reason, parents should not try to diagnose PLMD from sleep movements alone.
How Can You Tell When a Twitch During Sleep Is Concerning?
When you notice your sleeping baby have an unusual twitch, try to observe what happens before, during, and after it.
Ask yourself:
- When does it happen? Is your baby asleep, falling asleep, waking, or clearly awake?
- What does it look like? Is it a single brief jerk, a repetitive movement, or a whole-body response?
- Does it stop when your baby wakes?
- Does your baby remain responsive?
- Are there other changes? Look for changes in breathing, skin colour, eye movements, muscle tone, or behaviour.
These observations cannot and should not establish a diagnosis, but they can be very useful to a healthcare professional.
If possible, record a short video of a recurring movement. A video can show exactly what the movement looks like and may help a doctor decide whether further assessment is needed.
When Should You Seek Medical Advice?
Speak with your baby’s healthcare professional if you are concerned about unusual movements, particularly if they:
- Occur while your baby is awake
- Become frequent or progressively more intense
- Occur in repeated clusters
- Are accompanied by unusual eye movements
- Involve stiffening or unusual changes in muscle tone
- Are associated with changes in responsiveness or behaviour
- Occur together with changes in breathing or skin colour
- Are followed by unusual sleepiness or difficulty waking
A first suspected seizure should receive medical assessment. A seizure lasting more than five minutes, difficulty breathing, prolonged unresponsiveness, or significant changes in skin colour requires emergency medical attention.
Can A Baby Sleep Movement Monitor Help?
Because sleep twitches can be brief and easy to miss, movement monitoring can help parents observe patterns over time—including when movements occur and how frequently they happen. This can provide useful information to discuss with a healthcare professional if something seems unusual. However, movement monitoring can detect movement; it cannot diagnose the cause of a movement or distinguish a normal twitch from a seizure.
